Here we discuss our individual patient's problems through a series of inputs from the available global online community of experts with an aim to solve those clinical problems with collective current best evidence based input. 19 JAN 2022 On 11th jan , A 58 yr old male patient came to casualty   CHIEF COMPLAINT :  The patient came to casualty with complaints of shortness of breath since 1week  He also have cold and cough since 5 days  HISTORY OF PRESENT ILLNESS :  Patient was apparently asymptomatic since 1year back , he then developed shortness of breath , cough , pedal edema , decreased urine output ,  Distended abdomen , was taken to hospital and patient was diagnosed with renal failure and heart failure .  Patient was advised to undergo dialysis treatment but patient refused for it and was on conservative management .  Shortness of breath

Here we discuss our individual patient's problems through a series of inputs from the available global online community of experts with an aim to solve those clinical problems with collective current best evidence based input A patient came to casualty with chief complaint of pain in the abdomen since 2days .   History of past illness :  Patient was apparently asymptomatic 15years back and developed left sided pain diagnosed with renal calculi (for which pcnl surgery was done ) . 4months back patient developed yellowish discoloration in the eyes .  Pain in abdomen for which he was admitted in the hospital and managed consetvatively . Now patient is complaining pain in the  right hypochondrium region non radiating insidious onset  not associated with vomiting . Associated with fever low grade and intermittent relieves on medication

History of present illness :  Patient was asymptomatic 1week back. Patient started to develop high fever with temperature 102 degrees celsius  and chills . Had loss of appetite . Took 1 paracetamol tablet but conditions recurred . History of past illness :  No history of diabetes mellitus , hypertension , asthma , epilepsy . Personal history :  No habit of smoking nor alcohol No Appetite  Mixed diet . HISTORY OF PRESENT ILLNESS :  Patient was apparently asymptomatic 3years back when he has his first episode of right sided  upper and lower limb weakness and  was managed conservatively .  Then he again had his second episode of right side UL and LL  weakness 1year back  Managed conservatively. HISTORY OF PAST ILLNESS :  Patient is having hypertension frm past 1year. No diabetes mellitus No TB No epilepsy No asthma . PERSONAL HISTORY :  Patient occupation is cattle grazer. His appetite is normal Mixed diet. Bowel mo
